Beaver Dam Library Hosting Care Mapping Workshop

(Beaver Dam) The Beaver Dam Community Library is hosting Exploring Care Mapping this weekend. It is a hands-on opportunity to create a care map of your familyโ€™s strengths, in partnership with Children’s Resource Center-South, out of the Waisman Center, and with Wisconsin Medical Home: Childrenโ€™s Health Alliance of Wisconsin. 

A Care Map is a visual way to show all the people and services involved in caring for your child. Each map may look different depending on how much detail is put into them.  

For those that have children 12 and over, there will be a discussion on what a Transition Care Map might look like and what people choose to do when their child turns 18.  

The workshop is Saturday, May 11th, from 10am to 11am at the Beaver Dam Community Library. Registration is required. A link to register online is available HERE. Those interested can also call 608-265-8955.
