(Beaver Dam) Local health organizations are seeking community input as they work to develop a road map to better health for Dodge and Jefferson county residents. Beaver Dam Community Hospital, in collaboration with partners in Dodge and Jefferson County, is working to complete a community health needs assessment, which will be used as a tool to prioritize significant health needs and to create a plan for improving the overall health of the community. As part of the assessment, a Community Health Survey has been created to capture area residents’ opinions on the health challenges of the region. The survey is anonymous and takes less than seven minutes to complete. The deadline for completed surveys is Friday, April 26. Results will be made available at a Community Health Summit on May 9. Completed surveys can be dropped off at the BDCH Registration Desk just inside the main entrance, 707 S. University Ave., Beaver Dam. The survey is anonymous and takes less than 7 minutes to complete.
