John “Jack” Hazlewood
John “Jack” Farnsworth Hazlewood, age 77 of Brandon, passed away peacefully at home on Thursday, September 13, 2018.
Jack was born on August 1, 1941 in Ripon, the son of Ross F. and Grace E. (Frei) Hazlewood. On December 29, 1961, he was united in marriage to Carol Ann Schmuhl at Zion Lutheran Church in Fairwater, where they were both members.
After graduating from Markesan High School, Jack attended Layton School of Art in Milwaukee. He worked in the graphic arts and advertising for Kelley Company in Milwaukee, Lawn-Boy in Plymouth, and Gleason Reel in Mayville.
Jack was an active member of Memorial Lutheran Church in Glendale, Our Savior’s Lutheran Church in West Bend, and Zion Lutheran Church in Fairwater. He coached baseball and track in the Kewaskum School District. Painting pictures of landscapes was something he immensely enjoyed, as well as photography and digitally modifying photos. He was an avid Brewers and Packers fan.
Jack is survived by his wife, Carol of Brandon; children, Laura Lea (Tim) Streich of Oshkosh, Jandelyn Dawn (Michael) Plane of Greenbelt, MD, and John Farnsworth (Kimber Lee Cleveland) Hazlewood of New York, NY; grandchildren, Angelisa and Jonathan Plane, and Molly Streich; siblings, Anne Rasnick of Independence, KS, Mary (John) Siegle of Lynchburg, VA, Jane Soman of Fennimore, and Ross A. “Chip” (Lucy) Hazlewood of Markesan; and further survived by nieces, nephews, other relatives, and friends.
He was preceded in death by his parents; father- and mother-in-law, Lloyd L. and Marcelette M. Schmuhl; and brothers-in-law, Cline Rasnick and James Soman.
Visitation for Jack will be held on Monday, September 17, 2018 from 5:30 – 7:30 p.m. at Cornerstone Funeral & Cremation Services, 181 S. Main St., Markesan. Visitation will also be held on Tuesday, September 18, 2018 at Zion Lutheran Church, 100 Church St, Fairwater, from 10:00 a.m. until the time of the funeral service at 11:00 a.m. with Pastor Sam Hamilton-Poore officiating. Burial will take place at Mackford Union Cemetery, Town of Mackford.
The family deeply appreciates the care provided by Dr. Natalie Callander, Dr. Arjang Djamali, transplant coordinator Jolene Simmons, the staff at UW Hospital and Clinics, as well as the staff of Hometown Hospice.
