Charles D. “Chuck” or “Cooker” Cook, age 47 of Beaver Dam, went to his eternal home to be with his Lord and Savior on Thursday, July 26, 2018 at St. Mary’s Hospital in Madison after a long battle with diabetes since the age of 21.

Chuck was born to Lois Cook in Beaver Dam on October 15, 1970. He graduated from Beaver Dam High School and later worked at the Beaver Dam Senior Center and Dos Gringos restaurant. He was a member of St. Stephen’s Ev. Lutheran Church in Beaver Dam. An avid Green Bay Packers, Milwaukee Brewers and Bucks fan, Chuck also enjoyed going to neighboring communities with his Grandma and friends to play bingo, and always looked forward to their trips to Ho-Chunk Casino.

Chuck is survived by his mother, Lois Cook, of Beaver Dam; his uncle, Dean (Susan) Cook III of Fox Lake; his cousins, Jason (Jess) Cook and Christal (Cody) Bruesch; and is further survived by other relatives and friends.

He was preceded in death by his grandparents, Dean and Ruth Cook, along with other relatives.

Visitation for Chuck will be held on Wednesday, August 1, 2018 at St. Stephen’s Ev. Lutheran Church, 300 West St. in Beaver Dam, from 1:00 p.m. until the time of the funeral service at 2:30 p.m. with Rev. Paul Stratman officiating.  Burial will take place at Oakwood Cemetery in Beaver Dam.
